To all students under the opt-out mechanism, you are required to submit a notarized waiver before enrolling for the second semester of academic year 2024-2025.
You can secure a copy of the waiver at the Office of the University Legal Counsel at the TSU Gymnasium during the enrollment period from 7:00 AM to 6:00 PM or download and print the waiver on the TSU website under Downloads for Students > Accounting Unit > Opt Out Waiver Rev. 02 (https://tsu.edu.ph/downloads/for-students/).
You must also bring a photocopy of your TSU student ID with three specimen signatures. Meanwhile, students who are minors must submit a photocopy of their parent's or guardian's valid ID with three specimen signatures.
The Office of the University Legal Counsel will notarize the waivers with a minimal fee at the TSU Gymnasium from 7:00 AM to 6:00 PM.
